If you have a minor traveling with you of which you are not the parent or guardian of, or if only one parent is traveling with a minor then you will need to have a consent form signed and notarized. You will need to make one copy to take with you and one to leave at home just for reference if your original is lost. You can usually get things notarized for free at your local bank.
